Information for Day Surgery Patients

Pre-Admission Testing

You must have a pre-admission testing evaluation before any invasive procedure. What does this evaluation include?

Your pre-admission evaluation may include some of all of the following:

  • Medical History
  • Physical Exam
  • Education about your procedure
  • Anesthesia interview
  • Nursing Interview
  • Blood Test
  • EKG
  • X-Ray

Some of this information may be gathered by phone.

What do I need to bring with me to my Pre-Admission Testing appointment?

  • Insurance Cards
  • Referrals
  • List of all medications you are presently taking
  • Copies of testing done at a facility other than Temple
  • Your completed Patient Questionnaire

If you received care at a facility other than Temple, please be sure to bring a copy of any current medical records or tests. X-rays, EKGs, stress test results or any type of cardiology testing is especially important. You may eat and drink prior to your pre-admission evaluation.

Where is Pre-Amission Testing?
The Pre-Admission Testing Center is adjacent to Central Registration in the Main Lobby, first floor Rock Pavilion at 3401 North Broad Street (corner of Broad and Ontario Streets).
